Health Disparities

At the Center for Transformative Health, addressing health disparities is at the core of our mission. We are dedicated to understanding and mitigating the factors that contribute to unequal health outcomes among different populations, particularly those that are underserved or marginalized. Our work in this area focuses on identifying and addressing the social determinants of health, advocating for equitable care, and providing targeted education and support to empower individuals and communities.

Our Approach

Our approach to health disparities is rooted in a deep understanding of the complex social, economic, and environmental factors that influence health outcomes. We take a holistic view, recognizing that health is shaped not only by access to medical care but also by factors such as education, income, housing, and social support. By addressing these social determinants of health, we aim to create a more equitable healthcare landscape where everyone has the opportunity to achieve optimal health. Our programs are designed to be culturally responsive, ensuring that the unique needs and experiences of diverse populations are at the forefront of our efforts.

Our Focus Areas

  • By targeting health disparities, the center can direct resources and interventions where they are most needed, drive innovative solutions, and build trust within underserved communities, ultimately leading to more effective and inclusive health strategies. This focus not only aligns with social justice goals but also enhances the center’s role as a leader in advancing equitable healthcare and research.

  • Breast cancer is a critical area for advancing health outcomes and equity. Breast cancer is a leading cause of cancer-related morbidity and mortality, with disparities in incidence, treatment, and survival rates affecting different demographic groups.

  • Patient advocacy helps bridge gaps between patients and providers, addresses disparities in care, and promotes a more patient-centered approach to treatment and policy. By championing patient rights and fostering a supportive environment, we can drive improvements in care quality, enhance patient experiences, and contribute to more equitable and effective health outcomes.

  • Effective patient education enhances health literacy, fosters proactive engagement in managing health conditions, and improves adherence to treatment plans, leading to better health outcomes. By providing clear, accessible, and relevant information, we can help patients navigate their care more effectively, reduce health disparities, and promote a more informed and participatory approach to health management.

  • Social Determinants of Health

Understanding the broader social and economic factors that influence health is crucial to addressing health disparities. Our research and initiatives focus on the social determinants of health, such as housing, education, income, and access to healthcare. By identifying the root causes of health inequities, we work to develop and implement strategies that promote health equity and improve overall well-being for underserved populations.

  • Breast Cancer Awareness & Screening

Breast cancer disproportionately affects certain populations, particularly women of color. Our work in this area is dedicated to reducing disparities in breast cancer outcomes through targeted education, early detection, and access to care. We provide resources and support for breast cancer screening, treatment, and survivorship, with a strong emphasis on patient education and advocacy. Our goal is to ensure that every woman has the knowledge and tools she needs to take control of her breast health.

  • Patient Advocacy & Education

Empowering patients to advocate for their health is a key focus of our work. We provide education and resources to help individuals navigate the healthcare system, understand their rights, and make informed decisions about their care. Our patient advocacy programs are designed to support individuals from underserved communities who may face barriers to accessing quality care. Through workshops, support groups, and one-on-one consultations, we strive to equip patients with the knowledge and confidence they need to advocate for themselves and their families.

  • Community Engagement

Community engagement is central to our efforts to address health disparities. We collaborate with local organizations, healthcare providers, and community leaders to ensure that our programs are relevant, effective, and accessible. By working directly with the communities we serve, we can better understand their needs and develop interventions that truly make a difference. Our outreach efforts include educational campaigns, community workshops, and partnerships aimed at promoting health equity and reducing disparities.
